The Freedom of Information and Protection of Privacy Act (FIPPA) is a provincial law governing access to information and the protection of privacy. It has been applied to hospitals since January 1, 2012.
Under FIPPA, individuals have a right to request access to records that are in Holland Bloorview’s custody or control, dating back to January 1, 2007.
While FIPPA promotes public access to information, it also ensures that personal and confidential information is appropriately protected and that individuals’ privacy rights are upheld.
The Privacy Office of Holland Bloorview carries out the hospital’s commitments under FIPPA. Below you will find a wide range of information and resources related to freedom of information.

Personal Information Banks:
FIPPA requires Holland Bloorview to maintain an index of Personal Information Banks (PIBs). This index describes the personal information the Hospital collects to support its programs and services, where the information can be retrieved using an individual’s name or another identifier.
The Personal Information Bank includes the following:
- The title and type of the class of records
- The statute authorizing the collection of personal information
- The location of this class of records
- The format of this class of records
- A description of the type of information that is maintained in this class of records
- The uses and users of personal information maintained in this class of records
- A description of the types of individuals whose information is maintained in this class of records
- The retention and disposal (how long the information is kept)

Making a Request for information:
Before sending in a formal request for information, please contact the Privacy Office (see details below) to discuss what records you’re looking for. Many records can be provided directly by hospital departments, and you may not need to submit a formal request to access them.
