Preparing for Adult Life: The LIFEspan Service

A Partnership Initiative of Bloorview Kids Rehab and Toronto Rehab

Since disabilities don’t stop at age 18, Bloorview Kids Rehab has partnered with Toronto Rehab to develop The LIFEspan Service. This service helps create a smooth transition from pediatric to adult care.  It also aims to improve coordination of services throughout the client’s adult life.

The transition from pediatric to adult health care is often difficult for young people and their families.  Sometimes, it is hard to find adult health care providers who understand the needs of someone with a childhood-onset disability such as cerebral palsy, spina bifida, muscular dystrophy and brain or spinal cord injury that occurs during childhood. 

Who is this service for?

Currently, service is being offered to clients with Cerebral Palsy and Acquired Brain Injury.  There are plans to expand services to include a broader range of disabilities that started in childhood.

Who is on the LIFEspan service team?

Your health care team will include professionals based on your needs.  The team may include a(n):

  • Occupational Therapist
  • Physiatrist (a doctor specializing in rehabilitation medicine)
  • Physiotherapist
  • Social Worker
  • Speech Language Pathologist

How to access this service

When you are nearing 18 years of age, your team at Bloorview will talk to you about options for adult care.  They will tell you more about the LIFEspan service.

The LIFEspan service is located next door to Bloorview Kids Rehab at 345 Rumsey Road.
