Award Programs Information and Application Form

General Guidelines

• Limit of 3 students nominated per BRI Scientist (co-supervising would count as 1 student for each Scientist).
• Supervisor must agree to make adequate resources available to support the costs of each student’s research.
• One application for all of the Award Programs.

Student Research Proposal must:

• Focus on childhood disability
• Have relevance to the client population of Holland Bloorview Kids Rehabilitation Hospital  
• Be concordant with the mission of Holland Bloorview Kids Rehabilitation Hospital and the Bloorview Research Institute
• Be innovative, interdisciplinary and applied
• The supervisor must be a scientist in the Bloorview Research Institute or a full-time clinical staff member at Holland Bloorview Kids Rehabilitation Hospital with an appointment in the Bloorview Research Institute

Adjudication
A Graduate Student Award Programs Selection Committee will be appointed to review each application. The committee is comprised of scientists, adjunct scientists and post-doc fellows from the Bloorview Research Institute and clinicians at Holland Bloorview Kids Rehabilitation Hospital.

Duration of Award(s)
Effective period of the award is one year (September 1 to August 30). Successful candidates will be notified by Tuesday, July 31, 2012. Unspent funds must be returned to Holland Bloorview Kids Rehabilitation Hospital.

Payment
To activate the award, successful recipients must indicate (by letter, e-mail) acceptance of the award.

Acknowledgement
Award recipients are expected to acknowledge in all presentations and publications that their research was supported by their specific graduate award from Holland Bloorview Kids Rehabilitation Hospital. Recipients will be requested to write a thank you letter to the donor.

Deadline for Submission
Please submit a complete email application to jrappaport at hollandbloorview dot ca *(see Note below) by Friday, July 6, 2012, 4 pm. Materials received after this date will not be accepted.
