Collaborative practice among nursing teams

Improved communication among nurses improves the continuity of care for children and their families.

The aim of this initiative is build collaborative nursing teams, through the development of relationships, structures and processes required for developing and sustaining effective nursing teamwork.

Goals related to the implementation of this guideline include contributing to quality client and family care; enhancing interprofessional relationships and improving our primary nursing team model.

A primary nursing team model is a group of nurses that work together to care for a specific child.  As nurses work a variety of shifts - having a team of nurses that are assigned to work with a specific client helps to provide a comfortable environment with familiarity for clients and families. 

Throughout the implementation of this guideline, we are working to improve the primary nursing team model by setting guidelines for primary nursing team nurses and defining accountabilities and roles. 

Through our electronic health record at Holland Bloorview, we have provided the opportunity to communicate information related to admissions, rounds and family team meetings. Increase opportunities for communication will improve the continuity of care for children and their families.  

We are very excited to get started on innovative education approaches for nurses in October, which will include the use of our simulation lab and videos.  Once the education is complete, we will audit the use of the primary nursing team’s use of the electronic health record tools to evaluate communication and accountability.
