Holland Bloorview Earns Accreditation from Accreditation Canada

This fall, Holland Bloorview Kids Rehabilitation Hospital participated in Accreditation Canada’s national Accreditation program and is pleased to report that we have earned Accreditation with no conditions.

Our visit with surveyors from Accreditation Canada, a non-government organization that measures health organization services against national quality standards, was held from September 26-29th, 2010.

In their evaluation, surveyors reviewed the processes and infrastructure we have in place to support service delivery, and our performance in providing high quality, safe services. Their review included a new “tracer” methodology, where surveyors followed the path of clinical or administrative processes by “shadowing” and interviewing staff and clients across the continuum of care. For clinical processes, tracers followed what happened from first contact with clients to discharge. On the administrative side, they observed how administrative processes such as quality management are integrated into daily activities. The review also included team interviews, onsite tours, document review and interviews with our community partners, clients and families. These interviews left surveyors with a very positive impression of Holland Bloorview. Surveyors were quick to emphasize staff commitment, knowledge and enthusiasm, and the safety and cleanliness of our facility.

Overall,Holland Bloorview was found to be compliant in 651 out of 658 Accreditation Canada criteria (99 per cent!). We met the criteria for all Patient Safety Required Organizational Practices. There were 6 unmet criteria (with one criterion rated not applicable). We will continue the work that has been started to address the identified areas for improvement and to integrate this work with our quality improvement program.

Holland Bloorview was also commended for five leading practices that demonstrate high quality leadership and service delivery. Leading practices are recognized for being creative and innovative, demonstrating efficiency of practice, linked to Accreditation standards and adaptable by other organizations. The following leading practices were identified at Holland Bloorview:

  • Leveraging Information Technology: Advances and Innovations in Patient Safety – Commended for our fully realized electronic medical record and innovative leveraging of information technology for patient safety (e.g. ulcer management tool).
  • Music Therapy on CCC – Acknowledged for providing a creative and stimulating opportunity for clients with complex needs to participate in music through the use of specialized adaptive music technology at the bedside.
  • Passport to Preparedness – Commended as a clear, portable reference and action guide to help staff prepare for the H1N1 pandemic.
  • Spasticity Management in Children with Cerebral Palsy – Recognized for our comprehensive interdisciplinary evaluation and treatment for clients who have difficulty with spasticity.
  • ID ME! – Commended for our low-cost engagement campaign to help clients and their families become partners in safety and adverse event prevention. I.D. ME! encourages clients and families to prompt staff when two identifiers are not requested before an intervention.

Congratulations to staff, clients and families, our Board of Trustees, community partners and all who were involved in our accreditation process and our journey to excellence! It was a great year of preparation and learning with remarkable results.
