Holland Bloorview partnered with University Health Network-Toronto Rehab, to develop the LIFEspan Service. What is LIFEspan? LIFEspan means Living Independently Fully Engaged. The service is designed to help young people with childhood-onset disabilities successfully transfer from pediatric to adult systems. The team’s philosophy is based on Shared Management Model and Holland Bloorview’s Growing Up Ready framework to support the preparation of youth and families for transition to adulthood. The LIFEspan Service is available to youth who are at least 14 years old, are current clients of Holland Bloorview and have cerebral palsy or neuromotor difficulties. Topics of discussion include: health care providers involved, school/academics, funding, respite, community participation, community resources, and health care knowledge.
