The Hospital must provide records on request, subject to limited exemptions as provided in the Act.

Please note: Your health information cannot be accessed through the freedom of information process. You can request access or request a correction to your personal health record by contacting Health Data Resources at (416) 425-6220 ext. 3423.

Hospital staff will inform you if you need to submit a formal freedom of information request.
